FORMS

The forms accessible on this page are created in two different formats. One is a standard Microsoft Word™ format and the other is the Adobe™ Portable Document Format (.pdf). The .pdf format is the one used by the Administrative Office of the Courts (AOC) for all of their online forms. In order to read PDF files, you will need to download software called the Adobe™ Acrobat Reader. This is a free download that is easily done by clicking the yellow button below. That will take you to the Adobe web site where further downloading instructions are available.

To access the Microsoft Word forms, you will likely need Microsoft Word™ installed on your computer. However, with current conversion features on some other word processing software (e.g. WordPerfect), you may be able to access and convert the forms with alternate software.

I have provided links to two different sources of forms. The first link is to the Administrative Office of the Courts (AOC) forms available on the internet. All of these forms can be printed. More and more of the forms are now fillable online. For a search engine for all of the available AOC forms (both fillable and nonfillable), click on the button below. If you know the form number, it will aid your search. Otherwise, you can search using a keyword for the type of form you are looking for.

The second link is to forms used pursuant to the Local Civil Rules of the 30th Judicial District. One set of these forms is in the Adobe .pdf format the other set is in Microsoft Word format. All of these forms are fillable in one format or the other. Just click on the icon to the right of the form to which you want to go.

NOTE: If you intend to forward a form to me for electronic signature, please use either the Microsoft Word forms (see below) or forms created with WordPerfect. I cannot attach my signature to documents created in the Adobe Portable Document Format. I will convert the Microsoft Word and WordPerfect documents to the .pdf format and return them to you. - SJB
